Decentralized clinical trials in the trial innovation network: Value, strategies, and lessons learned

New technologies and disruptions related to Coronavirus disease-2019 have led to expansion of decentralized approaches to clinical trials. Remote tools and methods hold promise for increasing trial efficiency and reducing burdens and barriers by facilitating participation outside of traditional clinical settings and taking studies directly to participants. The Trial Innovation Network, established in 2016 by the National Center for Advancing Clinical and Translational Science to address critical roadblocks in clinical research and accelerate the translational research process, has consulted on over 400 research study proposals to date. Its recommendations for decentralized approaches have included eConsent, participant-informed study design, remote intervention, study task reminders, social media recruitment, and return of results for participants. Some clinical trial elements have worked well when decentralized, while others, including remote recruitment and patient monitoring, need further refinement and assessment to determine their value. Partially decentralized, or “hybrid” trials, offer a first step to optimizing remote methods. Decentralized processes demonstrate potential to improve urban-rural diversity, but their impact on inclusion of racially and ethnically marginalized populations requires further study. To optimize inclusive participation in decentralized clinical trials, efforts must be made to build trust among marginalized communities, and to ensure access to remote technology.
Fat and lean mass predict time to hospital readmission or mortality in children treated for complicated severe acute malnutrition in Zimbabwe and Zambia

HIV and severe wasting are associated with post-discharge mortality and hospital readmission among children with complicated severe acute malnutrition (SAM); however, the reasons remain unclear. We assessed body composition at hospital discharge, stratified by HIV and oedema status, in a cohort of children with complicated SAM in three hospitals in Zambia and Zimbabwe. We measured skinfold thicknesses and bioelectrical impedance analysis (BIA) to investigate whether fat and lean mass were independent predictors of time to death or readmission. Cox proportional hazards models were used to estimate the association between death/readmission and discharge body composition. Mixed effects models were fitted to compare longitudinal changes in body composition over 1 year. At discharge, 284 and 546 children had complete BIA and skinfold measurements, respectively. Low discharge lean and peripheral fat mass were independently associated with death/hospital readmission. Each unit Z-score increase in impedance index and triceps skinfolds was associated with 48 % (adjusted hazard ratio 0·52, 95 % CI (0·30, 0·90)) and 17 % (adjusted hazard ratio 0·83, 95 % CI (0·71, 0·96)) lower hazard of death/readmission, respectively. HIV-positive v. HIV-negative children had lower gains in sum of skinfolds (mean difference −1·49, 95 % CI (−2·01, −0·97)) and impedance index Z-scores (–0·13, 95 % CI (−0·24, −0·01)) over 52 weeks. Children with non-oedematous v. oedematous SAM had lower mean changes in the sum of skinfolds (–1·47, 95 % CI (−1·97, −0·97)) and impedance index Z-scores (–0·23, 95 % CI (−0·36, −0·09)). Risk stratification to identify children at risk for mortality or readmission, and interventions to increase lean and peripheral fat mass, should be considered in the post-discharge care of these children.

The latest collection of the most up-to-date research on matters of medieval military history contains a remarkable geographical range, extending from Spain and Britain to the southern steppe lands, by way of Scandinavia, Byzantium, and the Crusader States. At one end of the timescale is a study of population in the later Roman Empire and at the other the Hundred Years War, touching on every century in between. On 19 December 1946, the Irish President, Seán T. O'Kelly, signed the Unemployment Insurance Act into law. This innocuous-sounding piece of legislation has received very little attention from historians, but was of great importance to one section of post-war Irish society. Under its terms, Dublin and London entered into a special scheme whereby Irish men and women who had served with the British forces during the Second World War were allowed to claim British unemployment insurance payments, while still resident in the twenty-six counties of independent Ireland. Coming at a time of unemployment and economic slump in Ireland, this was of crucial importance to many exservicemen. This article will explore the background, negotiation and implementation of the unemployment insurance agreement, and will speculate on the reasons why the Taoiseach, Éamon de Valera, agreed to it. It will also examine the British side of the scheme and explore London's motives, both concrete and notional.

Health System Preparedness for Bioterrorism: Bringing the Tabletop to the Hospital

Objective:
To evaluate the acceptance and usefulness of a hospital-based tabletop bioterrorism exercise.
Design:A descriptive study of responses to a smallpox scenario delivered as a tabletop exercise in three modules.
Setting:A large, multi-institutional urban health System.
Participants:Healthcare workers representing 16 hospital departments.
Results:Thirty-nine (78%) of 50 invited employees from 4 hospitals participated. Key responses highlighted the importance of pre-event planning in intra-departmental communication, identification of resources for the dependents of health-care workers, clarification of the chain of command within the hospital, establishment of a link to key governmental agencies, and advanced identification of negative pressure rooms for cohorting large numbers of patients. Almost one-fourth of the participants described their hospital departaient as poorly prepared for a bioterrorism event of moderate size. At the conclusion of the tabletop, 79% of the participants stated that the exercise had increased their knowledge of preplanning activities. Seventy-nine percent of all participants, 94% of physicians and nurses, and 95% of participants from non-university hospitals ranked the exercise as extremely or very useful. The exercise was completed in 3% hours and its total direct cost (excluding lost time from work) was $225 (U.S.).
Conclusions:Tabletop exercises are a feasible, well-accepted modality for hospital bioterrorism preparedness training. Hospital employees, including physicians and nurses, rank this method as highly useful for guiding preplanning activities. Infection control staff and hospital epidemiologists should play a lead role in hospital preparedness activities. Further assessment of the optimal duration, type, and frequency of tabletop exercises is needed.

Warfare is one of the central themes of medieval history. Until now, however, there has been no journal dedicated specifically to this area. The Journal of Medieval Military History, the new annual journal of De Re Militari: The Society for Medieval Military History will remedy this situation by publishing top-quality scholarly articles on topics across the full thematic and chronological ranges of the study of war in the middle ages.
Medieval society was dominated by men who considered themselves more as soldiers than landlords, judges or administrators. More of society's resources went into fortifications than cathedrals; deeds of arms were a topic rivalled in literature only by love; and in many times and places the common people dreaded war far more than famine or plague. War was the greatest force in determining the evolution of medieval governments. Although the study of war, its conduct and its impact, has never been absent from medieval historiography, the past few decades have seen this field rise to new prominence.

Objective: The purpose of this study was to determine the prevalence of a range of mental disorders in an elderly community dwelling population in Dublin using AGECAT, a reliable and standardised computerised diagnostic system that has been previously validated in community epidemiological surveys.
Methods: A total of 451 individuals over the age of 65 in a GP group practice were interviewed using AGECAT.
Results: Approximately one in five subjects had a mental illness according to AGECAT criteria. The prevalence of depression was 13.1% and did not change appreciably with age. 5.5% of subjects met organic ‘case’ criteria, and the frequency of organic cases increased with age, approximating 15% in the over 80 year age group. Neurotic disorders occurred with very low frequency, representing 1.1% of the subjects tested.
Conclusions: These findings have implications for mentalhealth services planning at a local level, and underscore the utility of AGECAT as a tool for comparative crosscultural epidemiological surveys.

Philosophy—East and West, the book of the East-West Philosophers’ Conference held at Hawaii in 1939, adds to the growing evidence of a desire for rapprochement in academic circles between America and the Far East. The scope of the book is philosophical and rational, prescinding, that is, from religious questions as such, and seeking to formulate and to organise the common ground of Eastern and Western thought and thus to make more readily accessible to each what one possesses and the other lacks. The project arises from so manifest and natural a tendency—the desire of men to understand one another and so extend their understanding of the world—that any elaborate justification seems uncalled for, and in effect the majority of the Eastern contributors dispense with any such preamble. In the nature of the case the position of the Western philosophers is a more self-conscious one, and the justifications they put forward raise questions more important than the conference itself. It must be added that such a statement intends no disparagement of the book in which the proceedings of the conference are recorded, and is one in which the American contributors themselves would probably agree.
The first question, to which we may defer the answer while using it as a key to unlock others, is this: In what sense can we accept the statement that the traditions neither of the East nor of the West are wholly true or adequate to a full understanding of man? It is a proposition in which the Western and, at least, one of the Eastern contributors explicitly concur, and is in line with the objective of a “planetary philosophy“, in which the same amount of agreement is evident.
